The dissertation deals with the problem of determining new pathogenetically justified approaches to the diagnosis, prevention and treatment of abnormal uterine bleeding (АUB) in women of reproductive age with extragenital disorders. The study examination of 100 women with abnormal uterine bleeding (AUB) and accompanying extragenital disorders and 50 healthy women who underwent a medical examination. Research methods: clinical, laboratory, instrumental, hysteroscopy, echosonographic, dopplerometric, immunoenzymatic, morphogistological, statistical, curative. On the basis of a multicomponent study, the clinical and anamnestic, environmental factors, against the background of which AUB occur in women of reproductive age, were identified, and the structure of extragenital pathology in such patients was studied. By means of echographic, hysteroscopic and morphological researches it is set basic pathological structural changes of the endo- and myometrium. The important value of measuring of index of pulsation index is well-proven in the spiral and basale vessels of еndometrium, as a differential-diagnostic dopplerographic marker in the examination of patients with uterine bleeding. The doctoral deepened the scientific concepts of disorders in the pituitary-ovarian system, obtained data on hormonal imbalance. Levels of hormones and vitamin D were determined depending on the nature of structural changes of the endo- and myometrium in women with AUB and extragenital pathology. The etiopathogenetic significance of angiogenic factors, disorders of the humoral and cellular immune response, vitamin D deficiency in the occurrence of AUB in the presence of extragenital diseases in women of reproductive age was revealed. A highly sensitive and specific mathematical model for predicting the occurrence of uterine bleeding has been developed, which is based on the algorithm of a diagnostic complex with the determination of the hormonal profile, echographic and dopplerometric markers, taking into account hysteroscopic and morphological changes. Approaches to complex treatment and the selection of an individual appointment of appropriate therapy have been improved, which is based on the use of medical and diagnostic hysteroscopy, the achievement of hemostasis, the correction of hormonal, immune, micronutrient homeostasis, and somatic health in women with AUB and extragenital pathology. Treatment helps to optimize the state of the reproductive system, reduce the risk of developing emergency conditions and disease progression, and improve the quality of life of women of reproductive age.
