The identified mental pathology depends on the premorbid condition and the impact of HIV infection. In particular, neurotic and affective disorders were formed in premorbid mentally harmonious individuals after receiving information about HIV infection. The frequency of mature personality disorders is determined by unfavorable microsocial factors in childhood, adolescence and youth and pathological personality development after HIV infection. The impact of HIV infection on the patients mental health was manifested in significantly more frequent layering of organic, neurotic, affective disorders on existing personality disorders. The analysis of the results of the study of higher cortical functions and the emotional sphere revealed the following: 1) all areas of higher cortical activity undergo significant changes in HIV infection; 2) the most significant changes are noted in the field of memory, attention, visual, somatosensory, auditory gnosis, dynamic praxis, the ability to write, read, count and also in intelligence; 3) in the field of attention, changes are detected in the control group and significant violations in the research group; 4) minimal changes in the control group are presented in the following areas: language, reading, writing, intelligence, dynamic praxis and auditory gnosis; 5) changes in the emotional sphere prove the importance of the emotional 11 factor in the studied group (predominant memorization of emotional words), as well as a decrease of cognitive abilities in IIIIV clinical stages of HIV infection (decrease of memorization of emotional and neutral words). It was established that in the IIIIV clinical stages of HIV infection, the majority of patients were in a state of subdepression, which was accompanied by a high level of anxiety (compared to conditionally healthy individuals P<0.001). Scientific novelty of the obtained results. The level of HIV load in cerebrospinal fluid samples was determined in a prospective cohort study on a representative sample for the first time. Previously unknown features were revealed (the ratio of HIV load in blood and cerebrospinal fluid depending on antiretroviral therapy), characterizing the quantitative differences of HIV in two biological fluids of an infected organism. The level of HIV load in cerebrospinal fluid was determined depending on the presence or absence of clinical signs of CNS damage. It has been 14 confirmed that the amount of HIV in the cerebrospinal fluid can indicate the level of probability of the presence of HIVassociated damage to the central nervous system. Previously unknown features of the pathogenesis of CNS damage in HIV infection, as well as HIV replication in the infected body, were revealed. Practical significance of the obtained results. The justified possibility and expediency of using molecular biological studies of cerebrospinal fluid samples for the diagnosis of HIVinduced lesions of the central nervous system. The revealed regularities made it possible to develop an assessment of the diagnostic effectiveness of determining the level of viral load in CSF. The results of the study can be an indication for determining the profile of HIV pharmacoresistance in CSF, if the absence of HIV suppression in the sample of CSF is detected in a 6 months after starting therapy. The obtained data can be used for laboratory diagnosis of HIVassociated lesions of the central nervous system and monitoring of the effectiveness of HIV therapy.
