In this scientific and practical work there was elaborated the complex of diagnostic methods for early revealing of portal hypertension syndrome (PHS). The issues of management of pre-operative preparation of sick children and prophylactics of post-operative complications were solved. According to these propositions we suggested 4 variants of operation. Based on carried studies we elaborated the approach for choosing the method and determination of the scope of surgical intervention in children taking into consideration the leading mechanisms of gastro-esophageal bleeding development in the presence of portal hypertension. There was elaborated and implemented into the practice the new method of surgical intervention: selective decompression of gastro-esophageal zone using DSRA method combined with dissection of gastro-esophageal collateral venous blood flow without spleen ablating.
