Essay

It was carried out an examination and treatment of 74 patients with prostatic cancer, 96 patients with prostate benignant hyperplasia which underwent the prostatectomy. In accordance with the immediate results of the operations, three basic groups of patients were distinguished: group I - patients having no complications; group II - patients with postoperative enuresis; incoufinens of urine; group III - patients with permeability disturbance in the vesicourethral segment. It was determined a low preoperative tonicity of muscular structures of internal sphincter in patients with prostatic cancer (-48,1%), and in patients with prostate benignant hyperplasia (-53,7% compared with the normal rate), and further tonicity lowering with urinary bladder hyperreflexia development after prostatectomy, which is one of principal causes of postoperative enuresis incoufinence of urine. Postoperative permeability disturbance in the vesicourethral segment after the prostatectomy, in the main, is stipulated by a lowpostoperative detruser tonicity (-49,8%) in patients with prostatic cancer and in patients with prostate benignant hyperplasia, postoperative detruser tonicity and higher tonicity of tissue structures of the vesicourethral segment (-58,5%). To improve the results of treatment of postoperative complications against the background of traditional treatment, the method of targeted pharmacotherapy was used (M-cholinolytics - ephedrine and dephedrine) with the enuresis and M-cholinomimetics with alpha-adrenoblocking (omnic), with permeability disturbance in the vesicourethral segment in combination with stimulation of neuromuscular structures of the low urinary tracts with tonicity changed during the operation (prostatectomy). Due to the application of a special technique the treatment efficiency of postoperative enuresis after prostatectomy in patients with prostatic cancer was increased by 73,2% and in the patients with prostate benignant hyperplasia by 55%. The results of patients treatment with postoperativepermeability disturbance in the vesicourethral segment were improved respectively by 18% in patients with prostatic cancer and by 23% in patients with prostate benignant hyperplasia. A total preoperative and postoperative patients examination with prostate diseases is very important for the choice of surgical technique, as well as for forecasting and successful treatment of postoperative complications, for patients' social and psychological adaptation during the treatment of these complications.
