Hrem O. Optimization of pharmaceutical aid of patients with type 2 diabetes mellitus (a model of clinical pharmacists' activity)

Object - pharmaceutical care in type 2 diabetes mellitus; aim - scientific substantiation of clinical pharmacists' activity model in out-patients clinics and hospitals in a context of pharmaceutical care of patients with type 2 diabetes mellitus; methods - bibliosemantic, systematic, clinico-pharmacological, clinico-pharmaceutical, marketing analysis, pharmacoeconomic, modelling; novelty - clinical pharmacists' activity model was developed on out-patient and in-patient levels with the purpose of optimization of pharmaceutical aid for patients with type 2 diabetes mellitus; outcomes - unify program of identification, analysis, solving and preventing of drug-related problems and methodology of expert pharmacotherapy quality evaluation were developed; principles of rational hypoglycaemic drugs choice were determined; plans of multi-vector pharmaceutical care and drafts of clinical pharmacists' professional documentation were developed; implementation - into the work of Lviv's healthcare settings, into educational process in Odessa State Medical University and Danylo Halytsky Lviv National Medical University; branch - pharmacy.
