Perunsky V. Improvement of efficiency of surgical treatment in children with gastroshisis and omphalocele

Object – gastroschisis and omphalоcele in newborns; aim – The purpose is the improvement of treatment results in children with gastroschisis and omphalоcele on the basis of developed rational medical-diagnostic tactics and improved methods of a surgical intervention; methods – clinical, biochemical, immunologic, microbiological, morphological, anthopometrical, special complex, statistical; newelty – For the first time the comparative estimation of different ways of a plasty of a forward abdominal wall was conducted, most often arising postoperative complications are defined. The volume and methods of preoperative preparation, terms and ways of transportation are scientifically proved. For the first time the comparative estimation of morphologic structure dermal-aponevrotic flap was provided in congenital developmental anomalies of a forward abdominal wall. For the first time the objective way of definition of a vistsero-abdominal disproportion degree is developed and introduced in clinical practice, criteria of a choice of an operative measure are defined, new ways of a plasty of a forward abdominal wall in children with gastroschisis and omphalоcele are developed; results – Thus, at patients with gastroshisis and omphalocele which have received complex of the diagnostic, treatment and rehabilitational actions, the signibicaut depression of quantity of complications. The lethality level in the examination group has decreased on 30,8 % in comparison with patients of the first group, and on 18,7 % in comparison with second group; it has been itroduced into medical practice and studying process; sphere – medicine.
