Object - pigmented nevus of medium, big and gigantic sizes in children; aim - improvement of efficiency of treating children for benign nevus by means of optimization of various methods of plastic surgery of skin including expanded dermatension; methods - histological and clinical; newelty - Histological study of children’s pigmented nevuses of big sizes has been conducted and layerwise structure of a stretched flap, located next to the nevus, has been examined. Possibility of treatment strategy identification, depending on the depth of the nevus cells occurrence, has been established. A method of combined treatment (tangential severance using laser surgery) has been established and implemented. Clinical application of free dermatension flap, obtained from remote parts of the body has been proven. Application of different ways of replacing big skin defects in children has been optimized and advantages of expanded dermatension have been established; results - Conducted analyses of surgical treatment of children with big pigmented nevuses on application of various methods of plastic surgery, advantages of expanded dermatension have been identified. Such method makes it possible to obtain a piece of skin of any desired size and configuration. On application of expanded dermatension the biggest width-length ratio (1:4) of the piece has been achieved, and it is possible to obtain a free transplant. After cosmetic results upon application of various ways of pigmented nevus surgical removal had been studied, the biggest percentage of excellent results were identified on application of expanded dermatension (96,7%);Introduced - in medical practice, in educational process; branch - medicine.
