Ryvak T. Clinical and pharmaceutical component of the rational pharmacotherapy conceptual model in Health Care of Ukraine

Object – pharmacotherapy rationality; aim – scientific rationale and clinical development of pharmaceutical component of the conceptual model of rational pharmacotherapy in the Health Care of Ukraine; methods – systematic approach and analysis, bibliographic and semantic analysis techniques and other scientific information, structural and logical analysis, analytical, sociological (questionnaire), clinical and pharmaceutical, clinical and pharmacological, statistical, pharmacoeconomic, modeling; novelty – obtained new evidence-based outcomes that in complex solve important scientific problem, which concerns the prevention of abusive drug’s use, excessive drug’s use (polypharmacy), providing professional drug’s information has proved with proven clinical efficacy, and consequently – increase the quality, safety, clinical and cost effectiveness of drug treatment population, saving the state and the patient's inappropriate drug’s use to pharmacotherapy; results – has been developed and scientifically proven clinical pharmacy component of the rational pharmacotherapy conceptual model and ways of its Health Care implementation, particularly by the clinical pharmacist. Processed by the pharmaceutical care model of evidence-based methodology developed by the evidence search, analyze and organize information about the drug’s efficacy and safety, scientifically justified the introduction of the term "evidence-based pharmaceutical care." The estimation of current pharmacotherapy in clinical practice adapted and supplemented by the method of drug-related problems (DRP) management. Format formulary items as the standard model drug’s information, an important information management component and rational pharmacotherapy of State drugs formulary a new type of information unit were processed. Clinical and pharmaceutical message, as a new form of pharmaceutical intervention was powered, implemented in research and teaching processes of departments Danylo Halytsky Lviv National Medical University: clinical pharmacy, pharmacotherapy, and medical standardization and department of surgical postgraduate education; area – pharmacy.
