Essay

The analysis of diagnosis and treatment of 205 children with anorectal malformations was performed. The program of diagnostic measures which fully characterizes the anatomical type of this disease in patients of both sexes before and after the imposition of colostomy was developed and adopted. It was proved that protective colostomy is a necessary stage in the surgical correction of all (except the anal membrane) forms of anorectal malformations in children. Under the anal stenosis, rectum-perineal and vestibular fistulas the imposition of terminal single-barreled colostomy is suggested. For all other forms of anorectal malformations the optimal is formation of separate double-barreled colostomy. The indications to the selection of the type and level of colostomy imposition under this pathology in children was elaborated. It was established that the anatomical features of the structure of sigmoid promotes easier mobilization during colostomy in left parts of the colon what is comfortable and effective in infants. The new ways of colostomy in infants was developed and introduced, including using laparoscopy. The set of preventive surgical approaches in colostomy impose on the stages of anorectal malformations' correction in children on the basis of anatomical variants of this anomaly was elaborated and improved, what permitted to reduce from 21.78 % to 4.81 % the number of early and from 31.68 % to 5.77 % - number of late complications, to avoid tactical mistakes and mortality after such operations.
