The object of research: severe chronic obstructive pulmonary disease (COPD); purpose - improvement of the efficiency of treatment the patients with severe COPD based on the establishment of clinical, morphofunctional and immunological peculiarities in remission phase and their pharmacological therapy; methods: clinical, biochemical, ELISA, histopathological, electron microscopy, statistical; novelty: clinical, functional and morphological changes in severe COPD remission phase are determined, for the first time it was found that the use of roflumilast in addition to maintenance treatment from 3 to 6 months accompanied by the elimination of morphological restructution of the mucous membranes of the respiratory tract, the normalization of collagen IV type IL-4, IL-6, TNF-alpha, IFN-gamma, CRP and lysozyme levels in BAF. Field of application - medicine.
