Essay

Object of study: exacerbation of chronic bronchitis (CB); the aim - to increase the effectiveness of treatment of patients with CB by using in the complex treatment of endobronchial administration of nitric oxide (NO); methods of investigation: general clinical, radiological, functional, endoscopic, immunological, microbiological and statistical; the novelty: obtained scientific data that the inclusion in the traditional complex treatment of patients with exacerbation of CB endobronchial administration of NO promotes rapid and pronounced positive dynamics of endobronchial inflammation in the 2nd - 4th sessions and provides normalization of blood oxygen saturation (increase in oxygen partial pressure in capillary blood and saturation of blood hemoglobin with oxygen) by reducing the inflammatory process. For the first time it was determined that the use of endobronchial administration of NO leads to a pronounced rehabilitation of the lower respiratory tract with a decrease in the integral indicator of the total logarithm of microorganism concentration in bronchoalveolar lavage, mainly due to gram-positive bacteria. Supplemented scientific data that in 34.1% of patients with exacerbation of CB on the background of endobronchitis II-III degree of inflammation is detected concomitant gastroesophageal reflux disease (GERD), which is associated with a higher incidence of tracheobronchial tree dystonia and is accompanied by lower oxygen partial pressure in the capillaries blood. Proved the effectiveness of endobronchial administration of nitric oxide in the complex treatment of patients with comorbidity of CB / GERD, which leads to a marked decrease in microbial load of the lower respiratory tract with a decrease in the total logarithm of microbial concentration in bronchoalveolar. It is scientifically substantiated that the inclusion in the complex treatment of patients with exacerbation of CB endobronchial administration of nitric oxide administration allows for extending the clinical remission by 2 months.
