Essay

Object of study: bronchial asthma with the concomitant coronary heart disease; the aim – improving the comprehensive treatment of bronchial asthma (BA) against the background of coronary heart disease (CHD) taking into consideration endothelial dysfunction using the agonist of peroxisome proliferate-activated receptors gamma; research methods: clinical, instrumental (spirometry, bicycle ergometry, ultrasound vessel examination), questionnaires, laboratory as well as statistics; the novelty: it has been shown for the first time that patients with BA with the concomitant CHD present with increased systemic inflammation and disrupted functional state of the endothelium; the distribution of the PPAR-gamma-2 gene polymorphism among BA patients with the concomitant CHD in the Poltava region was studied for the first time. It has been found that the course of BA with the concomitant CHD is aggravated in subjects with genotypes Pro/Ala and Ala/Ala of the PPAR-gamma-2 gene; the study provided the evidence that the Pro12Ala genetic polymorphism of the PPAR-gamma-2 gene is an important unmodified factor in the pathogenesis of BA with the concomitant CHD; the inclusion of pioglitazone in the comprehensive therapy of patients with asthma against the background of CHD contributes to improved control of the disease, functions of external respiration, functional activity of the endothelium and quality of life; the effectiveness of the European Quality of Life Questionnaire EQ-5D and the specialized respiratory questionnaire of St. George's Hospital for the evaluation of the course of asthma against the background of coronary heart disease has been substantiated.
