Essay

The dissertation work is devoted to the solution of the urgent and important problem of the health care system that is the medico-social substantiation of the organizational and functional model of activity of a general practitioner/family physician (GP) in the system of provision a primary health care to urban population in conditions of formation a patient-oriented health care system in Ukraine. The conducted comprehensive medical and sociological research has established the imperfection of the existing organization of work of primary health care physicians, in particular GPs, to the challenges of the present day, conceptual approaches and strategic directions which were proclaimed by the World Health Organization (WHO) and led as a result to the necessity of substantiation and elaboration of an improved organizational and functional model of GP's activity in the system of provision a primary health care to urban population in conditions of formation a patient-oriented health care system in Ukraine for getting high-quality, effective and affordable health service at the primary level by patients. The scientifically substantiated and developed advanced, patient-oriented organizational and functional model of GP's activity in the system of provision a primary health care to urban population that contains additional organizational and structural components which are an integral part of success through the achievement of information and communicative competence of a primary health care physician and also evaluation of a satisfaction of the primary health care by patients. Including these components with a positive evaluation of the model as a whole by a group of experts (average score 9,77 ± 0,3 points, relative standard deviation 3,1) allows it to be recommended for implementation into practical GP's activity in the system of provision a primary health care to urban population.
