The dissertation is devoted to solving an urgent scientific problem, which is to experimentally substantiate the feasibility and effectiveness of using a new combined cream conditionally named "Dermalipoin", which included: α-lipoic acid, urea, olive oil, tea tree oil, PEG-400 for local treatment of wounds and burns in the 1st and 2nd phases of the wound process. The ointment is developed on an oil / water emulsion base with a moderate osmotic activity (200%) to prevent irritation on granulation tissue and ensure the removal of purulent exudate from the wound.
When studying the specific activity, for the first time it was found that the cream has pronounced reparative properties on models of linear cut, burn and squeezed stencil wounds, which manifest themselves in suppression of the inflammatory reaction and secondary tissue destruction, stimulation of plastic processes and acceleration of repair.
The advantages of the new combined cream "Dermalipoin" have been determined over the wound-healing preparations presented on the pharmaceutical market of Ukraine. In terms of the severity of the anti-inflammatory effect, the studied cream significantly surpasses the activity of the «Bepanten» cream and the «Rescuer Forte» balm on the models of inflammatory reactions caused by the action of the thermal factor, carrageenan and zymosan. The ability of «Dermalipoin» cream to reduce the permeability of the vascular wall caused by various phlogogens and to stabilize cell membranes exceeds the vascular and membrane stabilizing effect of thiotriazoline ointment. On the models of linear cut and burn wounds in rats in terms of planimetric and morphological parameters, the severity of the reparative effect of «Dermalipoin» cream significantly exceeds the activity of «Titriol» gel and «Rescuer-Forte» balsam. When studying the reparative effect of the investigated cream on a model of squeezed out stencil wounds in rats, according to hematological, planimetric, biochemical parameters, it was found that «Dermalipoin» cream is not inferior to the Rescuer Forte balm.
When determining the level of protein-synthetic activity of cells (epithelial cells of stratified squamous epithelium in the epidermal zone and fibroblastic cells in the margins of thermal damage) on the 15th day after modeling a thermal burn in animals of all experimental groups by assessing the content of RNP in the cytoplasm and DNP in cell nuclei, it was found that the cream "Dermalipoin" in comparison with the comparison drugs - balsam "Rescuer Forte" and gel "Titriol" more actively influenced the reparative processes, as evidenced by a significant increase in the protein-synthetic function of epithelial cells of stratified squamous epithelium in the epidermis zone and fibroblastic cells in edges of healing.
It was established for the first time that «Dermalipoin» cream has high antimicrobial properties against Staphylococcus aureus, Pseudomonas aeruginosa, Bacillus subtilis, is active against Escherichia coli, Proteus mirabilis. The therapeutic efficacy of the cream for experimental candidiasis infection in white mice has been proven, which, in terms of the microbial load in the candida lesion focus and the percentage of animal deaths, exceeded the antifungal effect of «Titriol» gel.
It was found that the cream according to the generally accepted classification of K.K. Sidorova belongs to the class of "practically non-toxic substances", both for cutaneous and intragastric administration, and also does not have a local irritating and allergenic effect.
