Borysov O. Recurrent nephrolithiasis in the conditions of accompanying chronic pyelonephritis (ethiology, pathogenesis, treatment and prophylaxis).

The dissertation is dedicated to the problem of urolithiasis in the conditions of accompanying chronic pyelonephritis under the use of different methods of stone's removing from upper urinary tract. Proved, that in the conditions of chronic-inflammation in kidneys the synthesis of overhigh concentrations of secretory IgA promote the development and recurrence of kidney stones.Substantiated the immune-biological criterium, determinated a susceptibility to recurrent lithogenesis (overhigh level of secretory IgA in urine and presence in structure of removed stone an adherent leucocites, bacterias and organic substances - carriers of IgA). The methabolic aspects of recurrent nephrolithiasis in the conditions of chronic pyeloneptritis were investigated. The integrative characteristic (morphological, structural, chemical and micro-elementary) of stones, developed in the conditions of chronic inflammation in urinary system was revealed.The dynamic of morphological and metabolic changes in renal tissues in exp erimental blant trauma (modeling, renal injury during ESWL) was studied.Taking into account the character of revealed immunological and methabolic disorders ininvestigated patients, with the aim of it's correction and recurrent nephro-lithogenesis prophylaxy in the conditions of chronic pyelonephritis, the own com-plexive approach in treatment of recurrent urolithiasis worked out and its efficacy was proved.
