Golik L. System of optimization of medical assystance in the Armed Forces of Ukraine (scientific grounding and realization

The dissertation is devoted to the issues of scientific grounding and implementation of the system of optimization of medical assystance to the servicemen of the Armed Forces (A.F.) of Ukraine and the veterans of military service under the conditions of reforming the military-medical service. It is established, that the present system of medical assistance in the A.F. of Ukraine and particularly its subsystem - primary, medical-sanitary assistance according to the medical, social and economical criteria under the new conditions - has proved itself as not enough effective. It is demonstrated, that implementation of new and improvement of the existing forms of organization of medical assistance in the A.F. of Ukraine famours effective use of personnel logistical and financial resources of military institutions of health care. The necessity of reorientation of the therapeutic link of the primary medical-sanitary assistance in the A.F. of Ukraine to the doctors of general practice - family medicine - is obvious. The new functional-organizational structure of the Main military clinical hospital - The Clinic of out-patients-polyclinical assistance - has been probated and implemented; also have been worked out the normative-judicial and organizational principles of its functioning. The qualitatively new system of treatment by stages in the chain "The Clinic of out-patients-polyclinical assistance. Main military clinical hospital. - The Hospital of finishing treatment and rehabilitation has been grounded up, developed and implenunted. Its principles aim to provide priority of development of out - patients' - polyclinical assistance on the basis of general - medical practice - family medicine, expansion of outside - the hospital forms of medical assistance, and profylactic work. On the basis of complex medical - sociological research separate components of improvement of out - patients'- polyclinical and hospital medical assistance have been scientifically grounded. The model of consolidated financing of military institutions of health care has been developed. The components of medical, economical and social effectiveness of the implemented qualitatively new system of optimization of medical assistance in the A.F. of UKRAINE have been determined.
