Yelagin V. Scientific Basis of Improving Medical and Genetic Help for the Population of Ukraine

The object is the system of medical-genetic care; the aim - scientific basis of measures that are directional to improving effectiveness medical-genetic help for the population; the methods - epidemiological and statistical, estimation of expert; Conception of the Special State Program "The Population of Ukraine Genofund Protection" has been created; integral prophylaxis system of genetically caused pathology has been created and implemented for heath promotion of Kyiv region population; the field is social medicine. Necessity of medical and genetic help developing was scientifically based in the network of the Special State Program "The Population of Ukraine Genofund Protection". Program Conception has been created. Demographic features of Kyiv Region population potential has been characterized. Fetuses reproductive losses were estimated, they made up 10.79 per 100 registered pregnancies during 1994-2003; within these 10 years this index has gradually decreased. Relative risk indexes in particular regions allowed to form special groups of regions where it is possible to improve medical care. Genetic pathology frequency has been estimated among born alive babies, stillborns and abortive fetuses according to medical indications (28.59‰, 49.69‰, 70.03‰ correspondingly). Geterozygosity level influence on population reproduction has been determined. Efficacy of pregnant women's ultrasound screening and newborns' phenylketonuria and hypothyrosis screening have been estimated. Risk factors of congenital pathology, spontaneous abortions and sterile marriages have been determined. Morbidity level of endocrine pathology among adults and children has been estimated in all regions of Ukraine. Level decrease of fetuses reproductive losses was registered in Kyiv region (OR=0.73 CІ 0.71-0.76 while comparing the data of 1999-2003 with the data of 1994-1998) thanks to implementation of integral prophylaxis system, including the genetic monitoring epidemiologic system.
