Zyukov O. Substantiation of structural and processing approach of inpatient healthcare quality management to oncologic patients in Ukraine

Object. System of organization of stationary medical help by the example of medical attendance of oncologic patients in radical surgical treatment. Aim. To ground and work out model of continuous improvement of quality of stationary medical help in Ukraine by means of systemic qualifying of processes and structure by the example of medical attendance of oncologic patients in radical surgical treatment. Methods systemic approach and analysis, bibliographical, historical, content - analysis, medical-statistic, epidemiologic ones, expert assessments, sociologic, conceptual modeling, organizational experiment, assessment of life span, investigation of life quality, psychologic methods. Theoretical and practical results. A model of administration of quality of medical help, based on an up to data concept of continuous improvement of quality was worked out and approved. Concentration on basing of optimal administrating decisions on answering the problems and achieving purposes linked with quality; structural-process approach to insertion of changes in the system of quality of medical help ( in the process of medical help, in the structure or process and in structure linked with it) is a principal peculiarity of the given model. It was proved that increase of successful results in the system of health service may be achieved by combination of reforms in processes and structure on various levels of medical help ( medical institutions - territorial system of public health service). Novelty. For the first time in Ukraine regularities and specific features of organization of stationary medical help rendered to oncologic patients on the national and regional levels and basic factors which influence the quality were investigated; the problems which are linked with organization- administrative aspects of quality were revealed. A model of continuous improvement of quality of medical help by means of systemic qualifying of processes and structure of medical attendance was grounded and worked out.
