The thesis is devoted to the problem of increase of efficiency of treatment of children with appendicular peritonitis on the basis of new approaches to diagnosis and development of the staging restoration treatment program. The basis of the work consists in treatment of 458 children with appendicular peritonitis in the age from 2 to 17 years. The thesis was executed in 2 stages: experimental and clinical. In the experiment on the model of local peritonitis it is proved that retroperitonial introduction of gentamycin results in substantial increase of its concentration both in the tissues of loose and dense inflammation (twice), and the use of galvanization promotes accumulation of preparation (1.3 times as much) in the tissues of loose inflammation. The data obtained during the experiment were used as the basis for elaboration of regional antibacterial therapy variants in treatment of local inflammation of the abdominal cavity. Based on the results of own experimental and clinical researches it was developed the complex program of restoration treatment of children with appendicular peritonitis by the staging use of natural and preformed physical factors under conditions of in-patient hospital, policlinic and on the stage of sanatorium-spa treatment. The complex program of staging restoration treatment of children with appendicular peritonitis (in-patient hospital - policlinic - sanatorium) was developed. The usage of this program increased the efficiency of treatment on the basis of new approaches and provided a "good" result of restoration treatment in 82,2 % cases and absence of "unsatisfactory" result.
