Object - purulent-septic diseases and sepsis in children; goal - improving the efficiency of treatment, prognosis and improve the diagnosis of purulent-septic diseases in children based on the study of clinical and morphological criteria and parameters of general and anti-endotoxin immunity; methods - clinical and anamnesis data of case history, X-ray, ultrasound, endoscopy, light microscopy, immunohistochemistry, immunologic indexes, statistics; novelty - based on conducted researches of pathogenetic role of general and anti-endotoxin immunity in children with purulent-septic diseases, including imunomorphology, first identified of early prognostic criteria's of development and course of septic process, which additionally describe main parameters of classification of septic process and expand the indications for immunotherapy of sepsis in addition to surgical treatment of acute purulent diseases, for which developed medical diagnostic algorithm which optimized tactics of surgical treatment in combination with immune-correcting therapy; results - surgical treatment with adequate sanitation of purulent foci in the affected organ, based on prognostic criteria developed using immunocorrection lead to rapid regression of clinical signs of disease, enhanced the clinical efficacy of therapy in 66.7% (p = 0.024) at SIRS, 51.2% (p = 0.019) - septic infection, 57.8% (p = 0.047) - sepsis, 83.3% (p = 0.026) - severe sepsis and septic shock in children with gram-negative infection in comparison to traditional methods of treatment; introduced - in the therapeutic and educational process; industry - medicine.
