Differential diagnostic features of cystitis and pyelonephritis were formulated according to clinical study of 404 children, based on exploring and comparing clinical manifestations, laboratory data analysis of blood, urine, lysosomal enzymes activity levels, antioxidant status. In-depth study of E. coli various pathogenic properties permitted to determine the bacteriological factors of pyelonephritis chronicity and recurrence in children. Dynamic observation of immunity in patients with pyelonephritis revealed the immune status components leading to chronical and recurrent behavior of disease. Structural and functional kidney characteristics study was conducted according to lysosomal enzymes activity levels in urine, beta2-microglobulin levels in blood serum and urine, albuminuria levels is allowed to regard these figures as predictors of unfavourable pyelonephritis course with gradual renal insufficiency development. Our research study enabled to design an algorithm in differential topical diagnostics of urinary infections in children on specialized medical care stage. Moreover, the efficacy of canephron N, furamag, sybutun, nucleinate sodium and imupret was proven, thus suggesting an improved model of pyelonephritis personalized therapy in children.
