Smiianov V. Medical and social proving of the model of health care management quality based on internal audit on the level of health care institution.

The thesis is devoted to the issues of scientific proving and developing the model of health care management quality based on internal audit on the level of health care institution. The basic elements of the developed model are two components. The first one is conditional permanent component, that include regulatory and methodological basis, financial and moral incentives to medical workers for rendering medical care of high quality, providing positive environment for the staff, concerning realization the principles of the continuous health care quality improvement concept. The second one is conditional variable component, the elements of which are analysis of health care institutions outcomes, setting priorities, audit type defining (clinical, managerial, financial and economical and others), multidisciplinary team teaching, plan and audit timing developing, audit register in health care institutions creating. The system classification of the internal audit types by analysis and test criteria to increase the effectiveness of its practical application was developed. Mechanism of feedback channels between the providers and consumers of health care services forming was suggested. This mechanism creates decision-making system focused on continuous health care quality improvement according to the needs and obligatory involvement of all the interested groups of people. It defines medical and social features of the model suggested. The developed model of health care quality management based on clinical audit mechanism, as a component of internal audit, was applied in health care institutions of Sumy city and Sumy region. The proposed model of health care quality management based on internal audit mechanism with the use of new tools of quality management is focused on applying the system of continuous health care quality improvement, adaptation of health care institutions work to new operating conditions of the branch and improving public health.
