The aim of the dissertation is to improve the diagnosis, treatment and prediction of superficial bladder cancer in view of influence of inflammation on its clinical course and justify the possibility of using of probiotics to prevent tumor recurrences. The study of 724 patients with bladder tumors revealed a negative effect of chronic lower urinary tract infections on the clinical course of the bladder cancer. In experiment on rats it was shown that chronic cystitis promotes chemically induced bladder cancer, while bladder instillation of probiotic strain Aerococcus viridans 167 has protective effect. The research on cell cultures of bladder cancer (Rt- 4, Tcc-sup) established antitumor activity of aerococci. The changes in indicators of local immunity and cellular markers of apoptosis, proliferation, cell adhesion in tumor tissue in patients with bladder cancer on the background of urinary tract infections were found. Antagonistic effects on pathogenic bacteria and immune-modulating effect of probiotic Aerococcus viridans 167 revealed. A diagnostic algorithm and statistical models based on clinical and morphological parameters (degree of differentiation, number of tumors, maximal tumor size, degree of bacteriuria) that allow increasing the effectiveness of cytological diagnosis and prognosis of recurrence of superficial bladder cancer developed.
