The thesis is devoted to more effective diagnosis and treatment of infertility in couple by study of new pathogenetic mechanisms of it's development and implementation of diagnosis and treatment algorithms for male infertility and identification of expedient duration of treatment depending on the associated female infertility. The new data on pathogenetic mechanisms of marital infertility in the male and female factors, when combined, which are in violation complex immunological, hormonal, genetic and immunohistochemical factors manifested in violation spermatological indicators. A medical and statistical analysis of the prevalence of male infertility in Ukraine traced significant fluctuations in male infertility levels between regions and areas, that proved necessity of accounting records improvement for a comprehensive registration of cases depending on the type of medical facility. Sensitivity and specificity semen parameters in terms of their reproductive value are established. The most sensitive indicators of potential male fertility were the following: DNA fragmentation spermal (Se = 0,94), the percentage of live sperm forms (Se = 0,71) and the total number of sperm in the ejaculate (Se = 0,69). When performing immunohistochemical studies demonstrates, that the higher the ubiquitin concentration in testicular biopsy material, the greater will be its indicator in cells ejaculate, so last way is more appropriate to use, it has a diagnostic and prognostic significance and has minimal trauma effect. The negative role of electro-magnetic radiation on semen parameters in vitro and in vivo is proved. The negative impact of radiation does not depend on the initial quality of the sperm (normospermiya or oligospermia) and expresses in all cases and correlates with the duration of mobile phone use.
