Essay

Object of research - Pulmonary embolism. Our aim was to optimizate the quality of diagnosis of pulmonary embolism in the way of studying the factors which lead to development of pathological state, the estimation of peculiarities of clinical course and hemodynamic characteristics under the influence of anticoagulant therapy. Methods of research: clinical; functional methods - electrocardiography, echocardiography with Doppler analysis; the method of questionnaire design of patients with the help of Ukrainian version of international questioner MOS SF-36 about quality of life; statistic methods. Results - long-term warfarin therapy (23,7±3,1) months in the dose of 3-5 mg is found to be associated with positive influence on hemodynamics of the left and the right portions of the heart which is characterized by decrease of systolic pressure in the pulmonary artery, significantly less incidence of concentric hypertrophy of the right ventricle, significantly decreased incidence of the most severe type of right ventricle overload, namely combined overload of the right ventricle by volume and pressure, also five factors are found, which increase the probability of lethal outcome in 5.5 times. The use of warfarin in the dose of 3-5 mg in complex program of therapeutic and preventive measures contributes to significant increase of absolute values of life quality characteristics according to physical and psychic status as well as general health which are objective signs of a person's adaptation level to social conditions.
