Object of research - nosocomial pneumonia, purpose - to optimize diagnostics and antibiotic treatment of nosocomial pneumonia in patients with myocardial infarction or unstable angina based on the analysis of major clinical and laboratory manifestations, results of microbiological monitoring in coronaru care unit considering pharmacoeconomic aspects, methods Research - clinical, microbiological, functional, biochemical, radiological, medical statistics and pharmacoeconomy; novelty - first studied the prevalence, spectrum of pathogens and peculiarities of nosocomial pneumonia in patients with myocardial infarction or unstable angina who are treated at the сокотфкн care unit. Results - found that nosocomial pneumonia developed in 14.6% of patients with myocardial infarction and unstable angina, the most common pathogens of nosocomial infections are: Staphylococcus spp. (32,1% of strains), Streptococcus spp. (24,2%), Enterococcus spp. (4,8%), non fermenting gram-negative bacteria (8,5%), Enterobacteriaceae spp.(20,0%), a history of diabetes, chronic obstructive pulmonary disease and smoking increases the probability of nosocomial pneumonia in such patients, the presence of cough with purulent sputum, whose intensity does not depend on time of day, increasing the intensity of breathlessness in spite of of heart failure treatment, level of C-reactive protein more than 55 mg / l, indicates acute infectious process in lungs.
