3. The object of research - chronic obstructive pulmonary disease; purpose - optimizing treatment of patients with chronic obstructive pulmonary disease stage II according to the installation of individual clinical and morphological patterns of pathogenic development and progression of the exacerbation phase of its course; methods: clinical, biochemical, HRP, electron, statistics; novelty - been shown that the development of COPD exacerbation phase already in the second stage there are damaged bronchial mucosa with a significant growth of connective tissue in her own plate, accompanied by reliable increases collagen type IV and decreased levels of lysozyme and sIgA in bronchoalveolar contents. Estimation dynamics endobronchial content of lysozime, sIgA, IL-4, IL-6, TNF, INF COPD at stage II in the elimination phase of sharpening and basic therapy of patients using tiotropiyu bromide. The priority is the data used are appropriate levels of collagen type IV and IL-6 in bronchoalveolar names as markers of progression remodelyatsiyi bronchial tree in COPD II stage. Been shown that prolonged use tiotropium bromide as a basic therapy of patients with COPD stage II is accompanied by the elimination of morphological characteristics of dystrophy or edema of epithelial cells of bronchial mucosa. Also on the background in the connective tissue colagenolisis own plate bronchial mucosa decreased the number of myofibroblasts, reliable normalization of levels of collagen IV type, IL-4, IL-6, TNF, INF in bronchoalveolar names and restore local protective barrier protection factors (sIgA, lysozime).
