Essay

The theoretical generalization and new decision of actual scientific prob-lem, namely - workings out of optimum method of treatment for persons with resistant to therapy alcohol dependence (RTAD) on the basis of researches of pathomorphism this disorder in the conditions of long action of inhibitors of acetaldehydedehydrogenase (IAADG) - is made in this dissertation. 180 per-sons (men by age from 20 till 66 years) with RTAD are surveyed. It was car-ried out dynamic, complex (clinical, paraclinical and psychometric) research of patients during the annual period of treatment. It is shown alcoholization pat-tern transformation (from initially continuous to intermittent) occurs under the influence of IAADG in persons with RTAD during 1-st trimester of antirelapse treatment. This creates preconditions for transition from daily administration of IAADG to administration "on demand" (only during days of the raised risk of alcohol use). Such variable mode of IAADG intake provides (during annual course and in comparison with isolated PSS): high efficiency of therapy (re-duction of "drunk days" quantity in 1,88 and 2,05 times (p<0,05) depending on type of IAADG) at essential reduction of frequency of adverse events; in-crease of patients adherence to treatment (in 3,63 times and in 4,50 times (p <0,05) depending on type of IAADG); reduction of patients needs in hospitali-zation (in 2,50 times and in 2,14 times (p <0,05) depending on type of IAADG); and also improvement of patients quality of life.
