The work is devoted to the complex study of clinical-psychopathological, pathopsychological, biorhythmologic features in patients with drinking forms of alcohol dependence (AD), taking into account the influence of heliogeophysical factors, optimization and introduction of a system of therapeutic and rehabilitation measures in this patient population. A total of 293 patients aged 19 to 63 years were examined, of them men - 217 (74.1%), women - 76 (25.9%). The first group consisted of 226 (77.1%) people with a constant type of alcohol abuse, the second group - 67 (22.9%) patients with an episodic type of abuse. An epidemiological study was conducted of statistical data of the Poltava Oblast Drug Dispensary and the A.F. Maltsev Poltava Regional Clinical Psychiatric Hospital for the 23rd eleven-year solar cycle (1997-2007). As a result of the study, the influence of heliogeophysical factors on the dynamics of morbidity in patients with drinking forms of AD has been shown to be manifested by an increase in the frequency of hospitalizations and mortality in the years of maximum solar activity and its significant changes (2001 and 2004). The incidence rate in this contingent of patients is closely related to circadian rhythm, which resulted in pronounced maxima of hospitalizations in March-April and November-December. The study of biorhythmologic features revealed manifestations of external and internal desynchronosis of circadian rhythm, which were much higher in representatives of the evening type of performance than in persons of the morning type. An integrative assessment of the types of alcohol abuse in patients with AD was made, taking into account clinical, social and pathopsychological indices, which allowed to determine the types of personality response and to take them into account when conducting therapy in this contingent of patients. The system of treatment and rehabilitation measures in patients with drinking forms of AD is optimized, combining the use of the laser therapy method based on chronomedicine in combination with pharmacotherapy in the first stage of stopping drinking states and the implementation of treatment and rehabilitation measures using a psychotherapeutic correction built on an integrative principle, depending on the type response in the second stage. The expediency of the proposed complex therapy is proved to create prerequisites for optimizing the treatment of this contingent of patients and to raise the level of psychological, social and professional functioning.
