Object - pathogenetic mechanisms of microcirculatory disorders in elderly patients with coronary heart disease (CHD); goal – to establish the role of endotoxemia, autoimmune and metabolic factors in the development of microcirculatory disorders in elderly patients with CHD and to evaluate the possibility of their correction of pentoksifilin; methods – biochemical, immune-enzyme, hemostasis, aggregate measurement, clinical and instrumental, statistic; results – the first time it is shown that the elderly with coronary artery disease recorded complex biochemical disturbances: endotoxicosis, inflammation, oxidative stress, angiogenesis disorder, an imbalance in the hemostatic system, dyslipidemia; it found that in elderly patients with CHD in contrast to the healthy elderly people has been an increase of spontaneous and induced apoptosis of mononuclear cells, suggesting a decrease in functional activity and depletion of domestic reserves cells; intensification of spontaneous apoptosis is associated with the deepening of endothelial dysfunction and decreased left ventricular ejection fraction in elderly patients with CHD; based on linear regression analysis established significant and independent predictors of the development of microcirculatory disorders and systolic left ventricular dysfunction in elderly patients with CHD; branch – medicine.
