Object - the functional state of the L-arginine / NO-synthase / arginase system and the course of oxidative processes in chronic kidney disease (CKD); aim – clarify the functional state of the L-arginine / NO-synthase / arginase system and the features of oxidative processes in patients with chronic renal failure of various etiologies under conditions of hemodialysis; methods – biochemical, laboratory, in-strumental, statistical; results – for the first time in patients with CKD of different etiology, the features of the L-arginine / NO syn-thase / arginase system in a complex with an assessment of its vasoactive levels in blood plasma and lymphocyte lysate were gharacterised; there was shown the dysfunction of the L-arginine / NO-synthase / arginase system had been detected in patients with CKD in uremia, after the hemodialysis the decrease in the level of uremia was combined with a more pronounced decrease in blood plasma concentrations of L-arginine, nitrite anion, asymmetric dimethylarginine and symmetric dimethylarginine , hydrogen sulfide, activity of arginase; in patients with CKD prior to the hemodialysis session, signs of activation of the processes of peroxidic lipid lipids were found with an increase in the levels of products of tiobarbituric acid (PTA) active products and a decrease in the level of vitamin C in blood plasma, and after hemodialysis, a decrease in the content of TBA active products and a deeper vitamin C deficiency; in patients with CKD revealed: an imbalance in the exchange of catecholamines; in blood plasma against a background of diabetic nephropathy associated with type 2 diabetes mellitus, changes in the main parameters of nitroso oxidative processes and the antioxidant defense system in directionality were similar to the corresponding changes in patients with CKD on the background of glomerulonephritis; in the lymphocyte lymphocyte the increase in the content of PTA active products was estimated, iNOS activity, decreased eNOS activity and L-arginine content compared with the control group, the hemodialysis session caused a significant de-crease in iNOS and eNOS activity, a decrease in PTA active products, L-arginine and nitrite -anion in the lysate of lymphocytes in patients with CKD; branch – medicine.
