The thesis is devoted to increasing the efficacy of complex treatment strategies of overweight patients with type 2 diabetes mellitus by cytokine profile regulation with liraglutide. It was found that type 2 diabetes mellitus in overweight patients is characterized by more pronounced changes to the carbohydrate and lipid metabolism parameters. It was discovered that poth pathological processes – type 2 diabetes mellitus as well as overweight – have a synergetic influence on proinflammatory cytokine levels, and more specifically elevation of these. It was determined for the first time that liraglutide possesses an additional pleiotropic effect and causes a statistically significant, definitive corrective influence on the levels of proinflammatory cytokines, thus decreasing the level of systemic inflammation. Additionally it was demonstrated that liraglutide had an impact on body mass index and quality of life parameters when used in complex treatment strategies for type 2 diabetes mellitus in overweight patients in comparison to metformin. An association was demonstrated between TCF7L2 C/T polymorphism presence and diabetes compensation parameters. The research demonstrated that presence of homozygous С/Т polymorphism of the TCF7L2 gene leads to the most pronounced negative influence on the efficacy of complex treatment strategies for type 2 diabetes mellitus in overweight patients with liraglutide use.
