Essay

The dissertation is devoted to the optimization of diagnosis and treatment of hyperplastic processes of endometrium by using modern diagnostic techniques. The thesis includes the clinical and statistical analysis, specific social and medical risk factors for hyperplastic processes of endometrium by three age groups; their separate and combined significance has been detected. With due account for the data of clinical and statistical analysis and instrumental and laboratory research has been developed and proved a new diagnostic diagnostics algorithm of endometrial pathology among the women through using modern functional and minimally invasive techniques. By means of immunohistochemical studies has been investigated the role of chronic endometritis and of the intratissual APUD-system in the genesis of hyperplastic processes of endometrium, and new pathogenesis links of this disease have been detected. Based on the clinical and instrumental examination and pathomorphological research has been developed and introduced into clinical practice the differentiated algorithm of staging diagnostic and therapeutic measures for optimization of medical care for patients with hyperplastic processes of endometrium. Applying a differentiated approach improved the treatment results of simple endometrial hyperplasia without atypia in the reproductive age by 24.9%, in premenopausal age by 20.9%, of complex endometrial hyperplasia without atypia in the reproductive age by 16.4% in premenopausal age by 19.4% . It was proved, that monopolar hysteroscopic or radio-frequency endometrial ablation can be recommended to the patients with non-atypical endometrial hyperplasia forms as alternative to hormonal treatment or a hysterectomy. It was detected, that the effectiveness of monopolar hysteroscopic endometrial ablation by women with non-atypical forms of endometrial hyperplasia makes 87.8%. Efficiency of monopolar hysteroscopic endometrial ablation by women in postmenopausal age with non-atypical forms of endometrial hyperplasia makes 95,0%.
