Essay

Theoretical generalization and new scientific solution of the problem is presented in the thesis, i.e. increasing the effectiveness of orthopedic treatment with metal-ceramic structures by improving and optimizing the odontopreparation algorithm, based on the patterns of morphological changes in the cellular composition of the gums. In the dynamics of clinical observations, the condition of the gums in relation to the peculiarities of the patient's dental status, the level of oral hygiene, as well as the type of odontopreparation for metal-ceramic orthopedic structures, and the qualitative characteristics of the cellular composition of the gums have been studied and analyzed. Some new data have been obtained regarding the possibility of early diagnosis and prognosis of the development of inflammatory changes of the gums in patients with metal-ceramic structures of dentures from these qualitative characteristics of different classes of gum epitheliocytes in a normal condition. A number of factors with different degrees of significance have been identified, which in the dynamics of clinical observations according to the percentage of the absorbed dispersion are decisive in the development of changes in the gums with different types of odontopreparation. Factor analysis of the investigated parameters of dental status in relation to the factors of teeth viability and the fact of injury of the gum margin at the time of odontopreparation have allowed to identify the main directions of significant changes in periodontal tissues and to determine key parameters for their implementation, prevention and complex of therapeutic measures. The algorithm of teeth tissues odontopreparation, based on realization of preparative, traumatic and periodontal factors, has been improved. The use of such an algorithm is convenient and it reduces the percentage of complications caused by this manipulation and, as a consequence, will allow to minimize the influence of factors that initiate changes.
