The scientific work is dedicated to solving the current scientific task of therapeutic dentistry – the development and implementation of differentiated approaches to the diagnosis, treatment and management of patients with lichen ruber planus of the oral mucosa, based on the regularities of the clinical course and detailing of pathogenetically significant factors. The indicators of dental health of female patients with LRP on OM were determined, and the relationship between the area of the lesion on OM and the parameters of dental status, psychological characteristics, the state of the vegetative nervous system, and metabolic indicators was analysed. According to the results of a cytological examination, in patients with LRP on OM, a violation of the differentiation process of the OM epithelium and changes in the cellular composition, which related to both quantitative composition and qualitative characteristics, were noted. Based on the results of the regression analysis, the parameters of the female patients’ dental, vegetative, psychoemotional and metabolic statuses, the key chains of development were determined for the first time, the concept of the pathogenesis of LRP on OM was proposed and the directions of significant changes in the female patient’s body were outlined, the leading criteria for their diagnosis and the planning of a therapeutic measure complex were determined.
The factors that influence the course of the LRP on OM were identified and mathematically substantiated. The first most significant factor, affecting the course of dermatosis, «dental», covered 64 % out of 100 %, the second most important factor ‒ «psychoemotional» covered 24.6 % out of 100 %, the third most significant factor – «metabolic» covered 11.4 % out of 100 %, which includes indicators of glucose level (8 %) and body mass index (3.4 %). On the basis of the obtained data, a questionnaire for determining the risk factors «Lichen ruber planus of the oral mucosa» was developed and implemented in clinical practice. In order to increase the level of awareness regarding the specifics of the clinical course of LRP on OM, to promote the individual motivation of female patients regarding the implementation of approaches and the algorithm of individual hygiene, to ensure orientation in the complex of treatment measures and to track the dynamics of treatment, the motivational diary «How to get rid of patterns in the mouth?» was developed and implemented. On the basis of the obtained data, the management strategy of female patients with LRP on OM using the principle of systematicity, differentiation and personal motivation was pathogenetically substantiated. Based on the results of the regression analysis of the best subset, prognostic criteria for the occurrence of LRP on OM were proposed, which include dental, psychoemotional, and metabolic factors. The problem of improving the diagnostic algorithms of LRP on OM got further development. A diagnostic algorithm, which includes a visual assessment of the discoloured areas of the LRP with further determination of the background colour on which the typical elements of the lesion are located, a characteristic of the type of pattern formed by the latter and an examination of the affected areas with the help of GOCCLES glasses, was developed. The issue of the LRP comorbidity, which requires close interdisciplinary integration with internists, gained further relevance.
